Job overview Fixed-Term Contract/Secondment available - monthsPLEASE BE ADVISED THAT THIS POST IS CURRENTLY ONLY OPEN TO EMPLOYEES OF CPFTWe are recruiting for a full time ( hours per week) Assistant care- co-ordinator on a six months Fixed Term Contract to support care provision and patient flow in our Intermediate Care Service.You will work across the county wide Intermediate Care Service supporting colleagues based in our South teams primarily .
You will be able to work flexible shifts patterns within our core service hours of am to pm, days a week.
Shifts in early and late patterns will be required to cover service am - pm, including weekends and bank holidaysThe vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital as part of the discharge to assess model.
Main duties of the job The job role is to assist the Care Co-ordinator within the Intermediate Care Service who will supervise and support the post holder to ensure referrals are managed in a timely and safe mannerThe job is focused on receiving new referrals into the relevant Intermediate Care Service, and assisting to co-ordinate and organise packages of care delivered by the local Immediate Care Service to support patients living in the community.To work closely with the relevant Intermediate Care Service's Team Lead and senior therapy staff to promote an multi-disciplinary approach to management of patient careTo work collaboratively with colleagues across the social care, health and the third sector to provide patient centred, integrated care Working for our organisation Cambridgeshire & Peterborough NHS Foundation Trust is a health & social care organisation dedicated to providing high quality care with compassion to improve the health & wellbeing of the people we care for, as well as supporting & empowering them to lead a fulfilling life.Our clinical teams deliver many NHS services not only via inpatient and primary care setting but also with the community.
