Keyresponsibilities include:Conducting routine and urgentappointments, both face-to-face and via telephone, ensuring high-quality care.Prescribing according to localand national guidelines.Handling home visits forroutine and emergency cases.Managing administrative tasks,such as letters, results, referrals, and emails, in a timely manner.Making appropriate referralsand managing related admin.Checking and signing repeatprescriptions to maintain care quality.Providing cover for colleaguesin emergencies, considering individual commitments.Participating in on-callsessions as per pro-rata hours.Supporting Extended Hoursservices, enhancing patient access.Collaborating with otherhealth and social care providers to deliver integrated care.Maintaining accurate consultationrecords using the EMIS clinical system.Offering clinical support tocolleagues, helping the team meet high standards in patient care andQOF/Enhanced Services targets.This is a dynamic role thatoffers both patient care and team collaboration in a supportive environment.

Job description Job responsibilities Clinicalresponsibilities:Inaccordance with the practice rota, as agreed, the post-holder will be availableto undertake a variety of duties including surgery consultations, telephoneconsultations and queries, visiting patients at home, checking andsigning/authorising repeat prescriptions and dealing with queries, paperworkand correspondence in a timely fashion.Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation.Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems.Screeningpatients for disease risk factors and early signs of illness in consultationwith patients and in line with current practice disease management protocols,developing care plans for health.Providingcounselling and health education.Admittingor discharging patients to and from the caseload and referring to other careproviders as appropriate.Recording clear and contemporaneous consultation notes to agreed standards.Collectingdata for audit purposes.Expectationto complete a mandatory induction.Compilingand issuing computer-generated acute and repeat prescriptions.Prescribingin accordance with the practice prescribing formulary (or generically) wheneverthis is clinically appropriate.Thepost-holder will be required to visit patients during surgery time in anemergency if clinically indicated.Thepost-holder will undertake a fair percentage of enhanced access sessions asallocated on the network rota.CommunicationUtilise and demonstratesensitive communication styles, to ensure patients are fully informed andconsent to treatment.Recognise the need foralternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Person Specification Experience Essential Experience of working in a GP training practice using EMIS Experience of dealing with a range of clinical conditions Experience of providing home visits Desirable Experience in triage Qualifications Essential Newly qualified GP (within 2 years and not having had a substantive post) with full GMC registration and license to practice Knowledge and Skills Essential Proven ability to evaluate the safety and effectiveness of own clinical practice Understanding of evidence-based practice Evidence of working autonomously, with some support and supervision Ability to effectively triage, assess and diagnose Requesting pathology tests and processing the results, advising patients accordingly Assessing and managing acute deteriorations/exacerbations of Long-Term Conditions Understands the importance of evidence-based practice Ability to assess and manage patient risk effectively and safely.
