Job summary Albion Medical Practice is a busy, well organised and supportive Practice based in Ashton-under-Lyne.
We have 10,000 patients and are looking for an experienced Part-time Advanced Nurse Practitioner to join our friendly, highly motivated Practice Team.
Main duties of the job We are looking for an enthusiastic and experienced AdvancedNurse Practitioner to work 3 days per weekYou will be working with a friendly, approachable and supportive team of5 Partners, 2 Salaried GPs, 1 ANP, 2 Practice Nurses, Nursing Associate,Pharmacist and HCA.

Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
Delivery of care in line with NSF, NICE guidelines and evidence-based care.
Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
Co-ordinate the planning and delivery of care ensuring patients and carers/relative are involved.
Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
Request diagnostic investigations and interpret within agreed protocols eg, x-rays, blood tests, scans etc Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
Person Specification Qualifications Essential Nurse Practitioner Qualification Independent Prescribing Certificate Experience of Chronic Disease Management Desirable Experience of working in a Primary Care environment preferable Experience Essential We are looking for someone with experience in: Minor illness Chronic Disease management (Asthma, COPD, Diabetes) Prescribing and reviewing medications appropriate to patient needs Desirable Experience of working in primary care.
Experience of working in a GP Practice.
Have a good understanding of QOF
