Job summary We are looking for an Advanced Nurse Practitioner to join our friendly experienced team at our Upwell Health Centre.
The role will involve working independently as an autonomous practitioner providing clinical assessment, diagnosis and treatment for our adult and paediatric patients with undiagnosed and existing conditions.
You will be working alongside our clinical team of GPs, Nurses, Pharmacists, Physiotherapists, Health Care Assistants and other associated roles to provide safe, holistic care to our patients.
This will include face to face and remote appointments.

About us Upwell Health Centre is a well-established, caring, forward-thinking practice in the pretty village of Upwell which is close to Wisbech, Downham Market and Ely, and is convenient for Peterborough and Kings Lynn.
We are a dispensing practice with a list size of 11000 patients.You will be part of a supportive team of 6 GPs, ANP, Specialist LTC Nurse, an excellent nursing team plus an experienced administration and management team.We are an active member of the Fens & Brecks PCN which includes clinical pharmacists, a home visiting team, social prescribers and first contact physiotherapists.

Person Specification Clinical Essential Competent clinical skills with a knowledge of chronic disease management.
Able to use analytical skills in practice.
Experience in minor illness/injury.
Excellent interpersonal and organisation skills.
IT experience.
Qualifications Essential RGN Recognised 3 year Nurse Practitioner Qualification to include autonomous practice.
Independent Nurse Prescriber.
Desirable Chronic Disease Management - Asthma, Diabetes, Chronic Obstructive Pulmonary Disease, Coronary Heart Disease, Stroke, Family Planning Experience Essential One years experience post Nurse Practitioner Degree.
Experience of multi-disciplinary working.
Able to demonstrate high level of personal professional autonomy and accountability.
Work using advanced clinical skills.
