Job summary Byron Medical Practice are looking to recruit an Advanced Nurse Practitioner to join our friendly team.
We are looking for a highly motivated individual, ideally with experience of working in primary care.The post is Full or Part Time.
Working hours are negotiable and will ideally be 20 to hours per week.Informal visits are welcome and can be organised by contacting the Practice Manager.
Main duties of the job You will be based within Byron Medical Practice and will holddaily clinics seeing patients predominantly face to face but occasionally conductingtelephone consultations.
You will berequired to provide some home visits to patients where appropriate.The post holderwill demonstrate courageous and critical thinking, they will be a caring,compassionate and committed experienced nurse who, acting within theirprofessional boundaries, will provide care for the presenting patients frominitial history taking, clinical assessment, diagnosis, treatment andevaluation of care.
They will demonstrate safe, competent clinicaldecision-making and expert care, including assessment and diagnostic skills,for patients of all ages within general practice.
The post holderwill commit to demonstrating critical thinking in the safe clinicaldecision-making process.
They will communicate and work collaboratively withthe general practice teams to meet the needs of patients, supporting thedelivery of policy and procedures, and providing nurse leadership as required.
About us We are a friendly, well-organised 3 Partner Practice serving around 8,500 patients.
We are also part of 5 practices that make up the North Easington Primary Care Network which has an excellent track record for collaboration, care and support.
Our Practice Team includes 5 GPs, 2 Nurse Practitioners, 2 Pharmacists, 3 Practice Nurses, 2 HCA's and a supportive administration team.
We have regular clinical meetings, practice meetings and MDT meetings as well as Protected Time in / time out sessions.
Support and mentorship will be provided and we actively encourage Specialist clinical interests and continued professional development.
Job description Job responsibilities Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis both in practice and within patients own homes.
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate) Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children Person Specification Qualifications Essential Registered first level nurse MSc or equivalent Digital Badge Relevant nursing/health degree Mentor/teaching qualification Clinical supervision training and experience Extended/independent nurse Independent prescriber Experience Essential Significant post registration experience Recent primary and community nursing experience Nurse-led management of minor illness, minor ailments and injuries Nurse-led triage Compiling protocols and clinical guidelines Clinical Leadership Audit Research Nurse prescribing Home visits Desirable Experience of working in primary care an advantage SystmOne experience an advantage
