Job summary This is an exciting new ANP role, working for the Newbury Street Practice basedat Wantage Health Centre.
The role will be predominately focused on same day,medically urgent patients.

They can independently prescribeappropriate medication, evaluate or refer to other specialists if necessary.ANP's in this role will focus solely on medically urgent patients.This is a fiull time position and we are flexible in working hours Mon - Fri About us This role will join a team of GPs, ECP, PA, Practice Nurses, NurseAssociate and HCAs.
The role will be based at Newbury Street Practice.We are based in the Health Centre, which has just been recently extended.
We serve approx 16,000 patients.There is free parking for staff.

Manage patients presenting with a range of acute and chronic medicalconditions, providing subject matter expert advice2.
Implement and evaluate individual specialised treatment plans forchronic disease patients3.
Identify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to the patients health4.
Provide advanced, specialist nursing care to patients as required inaccordance with clinical based evidence, NICE and the NSF5.
Undertake the collection of pathological specimens?6.
Request pathology services as necessary7.
Process and interpret pathology and other test results as required?8.
Maintain accurate clinical records in conjunction with extantlegislation9.
Ensure SNOMED CT codes are used effectively10.
Chaperone patients where necessary11.
Prioritise health issues and intervene appropriately12.
Support the team in dealing with clinical emergencies13.
Recognise, assess and refer patients presenting with mental healthneeds14.
Be an extended and supplementary prescriber, adhering to guidance15.
Support patients in the use of their prescribed medicines or overthe counter medicines (within own scope of practice), reviewing annually asrequired16.
Contribute to practice targets (QOF, etc.
), complying with local andregional guidance17.
Liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients, etc.)18.
Delegate clinical responsibilities appropriately (ensuring safepractice and that the task is within the scope of practice of the individual)19.
Support the clinical team with all safeguarding matters inaccordance with local and national policies20.
Understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately21.

review and process data using accurate read codes in order to ensureeasy and accurate information retrieval for monitoring and audit processes.SupervisionThe postholder will have access to appropriate clinical supervision andan appropriate named individual in the GP Practice to provide general adviceand support on a day-to-day basis.
Person Specification Experience Essential Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
Essential clinical skills including phlebotomy, ECGs, urine analysis and swab taking.
Working knowledge of Microsoft and GP practice and prescribing data monitoring systems.
Ability to write comprehensive clinical notes, implement and evaluate care plans.
Be aware of data protection (GDPR) and confidentiality issues particularly within a GP practice.
Desirable Mentorship or supervisory skills training.
Cognitive behavioural and motivational interviewing approaches / skills.
Knowledge and Skills Essential Ability to evidence a sound understanding of the NHS principles and values.
Ability to analyse and interpret complex/ often incomplete information, pre-empt and evaluate issues, and recommend an appropriate course of action to address the issues.

Qualifications Essential Registered Nurse with Nursing and Midwifery Council Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
