Job summary Heart of Bath Medical Partnership are looking for a part timeAdvanced Nurse Practitioner.
We are a Practice based across 2 sites in the Cityof Bath.
We have a diverse team of clinical and non-clinical staff who worktogether to provide care for our 26,500 patients.The post holder will be an experienced Nurse Practitioner who,acting within their professional boundaries, will provide assessment,diagnosis, and treatments to acute episodes of minor health conditions as wellas minor injuries.The ANP will be expected to see patients autonomously providingface to face, Telephone consultations within the practice and home visitswithin the patients residence and demonstrate the use of critical thinking inthe clinical decision-making process.You will work collaboratively with the general practice team andmeet the needs of the patients, supporting the delivery of policy andprocedures and providing Nurse leadership as well as promoting self-care to ourpatients.DBS checks are required for this position.Competitive rates of pay Main duties of the job Main Duties:Assess, diagnose, plan & implement care for patients.
Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
Diagnose and manage both acute and chronic conditions, intergrating both drug and non-drug based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs in accordance to National and Local guidelines.
Work with patients in order to support compliance with and adhere to prescribed treatments.
Provide information and advice on prescribed and over the counter medication regarding regimens, side-effects and interactions.
Prioritise health problems and intervene appropriately to assist the patient in all situations.

Person Specification Experience Essential Experience within primary care Desirable Triage experience would be an advantage.
