Job summary Heartbeat Primary Care CIC have recently formed a General Practice Urgent Care Hub and are looking for an ANP to support facilitate the contract.
The successful ANP will work across three local practices in the Hambleton North PCN area.
The hours will be 16 per week spread across 4 days, days to be agreed.
This is a fixed term contract for 1 year.To find out further information or to apply, please send in your CV to: Main duties of the job To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team.
The duties will include all tasks normally undertaken by an experienced ANP and in addition any roles agreed between Heartbeat Primary Care CIC and local doctors as appropriate, having regard to current training.
About us Heartbeat Primary Care CIC is a GP federation servicing twenty-one practices across the localities of Hambleton, Richmondshire and Whitby.
It was formed in 2014 to facilitate and encourage joint working between practices and with other partners.
It has forged close working relationships with all member practices across the locality and continue to support the development of relationships across primary care.
Job description Job responsibilities You will play a vital role in assessing, diagnosing, planning, and implementing care for the new Urgent Care Hub contract.Job Description: Conduct both face-to-face consultations in clinics, and telephone consultations.
See patients of all ages Treat minor illness and/or Injury trained Prescribe and review medication, ensuring adherence to treatment plans.
Manage cases autonomously Play a key role in service development and quality improvement initiatives to enhance care delivery.
Person Specification Experience Essential Significant experience in working as an ANP within primary care Ability to work autonomously Other Essential Full valid driving licence and have access to travel to various locations in the local area.
Qualifications Essential Hold MSc in Advanced Clinical Practice NMC Registered Degree at 2:1 level or above Possess independent prescribing qualification
