Advanced Nurse Clinical Practitioner

Job summary Rayleigh and District Primary Care Network are looking for a dedicated and experienced advanced nurse practitioner to join our team on a full time basis and contribute to our mission of delivering exceptional healthcare services to our community.
Based in our Rayleigh practice, you will play a pivotal role in delivering advanced clinical care to our patients.
You will work autonomously, providing assessment, diagnosis, treatment, and management of a wide range of conditions, both acute and chronic.
Due to home visits the applicants will need to hold a valid full drivers licence.
Main duties of the job Conduct comprehensive patient assessments, including history taking, physical examinations, and ordering diagnostic tests.
Diagnose and manage both acute and chronic conditions, making appropriate referrals where necessary.
Prescribe medications and treatments in accordance with current guidelines and best practices.
Provide expert advice and education to patients and their families regarding treatment options, preventative care, and self-management.
Collaborate with the multidisciplinary team to develop and implement care plans that meet the individual needs of patients.
Participate in the development of clinical protocols and pathways to improve service delivery.
Lead on quality improvement initiatives within the PCN.
Engage in professional development and training to maintain and enhance your clinical skills and knowledge.

Person Specification Qualifications Essential Master's in advanced clinical practice or an equivalent MSc or NHS England Centre for Advancing Practice Digital Badge (or working towards it).
Active NMC registration and licence to practice.
4 years experience as an advanced nurse practitioner, working at a masters level that encompasses the four pillars of clinical practice, leadership and management, education, and research.
Possess a prescribing qualification.
Experience Essential Driving licence.
