This is a growing work stream which in time will likely encompass primary care referrals and grow to cover other acute medical presentations and acute services.Care Home FallsThis very successful work stream provides care homes and nursing homes within NHSGGC direct access to the ANP team in the FNC.
The ANP will support care staff over video consultation to provide initial assessment and management of residents who have fallen in the care home.
