What We're Looking For: Registered Nurse (RMN/RNLD/RGN): Valid NMC registration is required.
Non-Medical Prescriber: Independent prescribing qualification required.
Proven ACP Experience: Experience as an Advanced Clinical Practitioner, preferably within mental health or rehabilitation.
Clinical Competency and Case Management: Strong skills in managing complex cases with a patient-centred approach.
Effective Communication: Excellent interpersonal skills for working with service users, families, and colleagues.
Commitment to Professional Development: Dedicated to evidence-based practice and continuous learning.
Key Responsibilities: Patient Assessment and Care Planning: Conduct initial assessments of new admissions to determine appropriate care, working under the guidance of the Consultant Psychiatrist and in collaboration with the ward team.
Documentation and Reporting: Prepare discharge summaries, reports, and other essential documentation.
Clinical Leadership and Supervision: Lead learning activities, clinical supervision, and participate in clinical governance.
Physical Healthcare and Emergency Support: Support physical health needs and provide emergency care as required.
Prescribing and Phlebotomy: Undertake prescribing and phlebotomy within your scope of practice and competency.
Risk Assessment and Safety: Conduct risk assessments, adjust observation levels, and ensure care remains as least restrictive as possible.
Liaison with External Agencies: Act as a liaison with external consultants, GPs, and case managers to coordinate comprehensive care.
