Main duties of the job You will be able to expertlymanage urgent community referrals, from our GPs, Hospital and Home andcommunity nurse colleagues.
You will manage these cohort(s) of patients andpresenting complaints, providing care for the presenting patient from initialhistory taking, clinical assessment, diagnosis, treatment, and evaluation ofcare, demonstrating safe, competent clinical decision-making and expert careevidence-based care.To be a clinical expert andsenior clinician in the field of Adult Community Care.
