Job Summary
The post holder will demonstrate a high level of expertise within the specialist service providing advice, education, and support to staff, patients, their families, and carers. The post holder will be practicing autonomously as an advanced clinical practitioner within the designated specialty area to provide patient-centered clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice throughout the directorate. The post holder will support new ways of working that emphasize a more efficient and patient-focused service and will ensure the safe treatment, referral, and discharge of patients with undifferentiated and undiagnosed presentations in their area. Responsibility for leading the ongoing development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols, and guidelines in collaboration with multidisciplinary colleagues.
Main Duties of the Job The post holder will demonstrate courageous and critical thinking and is a caring, compassionate, and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. Job Responsibilities Assess, diagnose, plan, implement, and evaluate treatment, interventions, and care for patients presenting with an undifferentiated diagnosis from within your sphere of competence. Clinically examine and assess patient needs from a physiological and psychological perspective and plan care accordingly. Assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs. Utilize your clinical supervisor/mentor and other clinical staff appropriately to ensure safe and effective patient care, demonstrating awareness of your own limitations. Provide safe, evidence-based, cost-effective, individualized patient care within the surgery, patients' own home, or other environments where patient care is carried out. Refer patients directly to other services/agencies as appropriate, utilizing practice and local guidelines in a timely manner. Proactively identify, diagnose, monitor, and manage treatment plans for patients at risk of developing a long-term condition, as appropriate. Diagnose and manage acute conditions, integrating both drug and non-drug-based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within own scope of practice. Utilize Patient Group Directions for the administration of medication as appropriate. Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed and over-the-counter medication on medication regimens, side effects, and interactions. Prioritize health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that promote healthy living and apply principles of self-care. Recognize, assess, and refer patients with mental health needs as appropriate. Communicate with and support patients receiving bad news. Person Specification Knowledge/Skills/Competencies Essential Awareness of accountability of own role and that required of a Registered Practitioner. Qualifications Essential Registered Nurse (NMC) or Registered Paramedic (HCPC) or Registered Physiotherapist (HCPC), Registered Dietitian, Registered Podiatrist, or Registered Pharmacist. Minor illness qualification at degree level assessed by OSCEs. Current registration. Relevant degree. Independent non-medical prescriber or working towards it, or proven ability to use Patient Group Directions. Desirable Undertaking a course in Advanced Clinical Practice at Masters level.
