Job summary The post holder will demonstrate a high level of expertise within the specialist service providingadvice, education and support to staff, patients, their families and carers.The post holder will be practising autonomously as an advanced clinical practitioner within thedesignated speciality area to provide patient-centred clinical care.
This will encompass the skills ofadvanced clinical assessment, examination, diagnosis and treatment within an agreed scope ofpractice throughout the directorate.The post holder will support new ways of working that emphasises a more efficient and patientfocused service, and will ensure the safe treatment, referral and discharge of patients withundifferentiated and undiagnosed presentations in their area.2Responsibility for leading the on-going development of clinical practice and standards of care withinthe service, including the development of policies, procedures, protocols and guidelines incollaboration with multidisciplinary colleagues Main duties of the job The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process.
They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Job description Job responsibilities Assess,diagnose, plan, implement and evaluate treatment, interventions and care forpatients presenting with an undifferentiated diagnosis from within your sphereof competence.
Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective and plan care accordingly.Assess,diagnose, plan, implement and evaluate interventions/ treatments for patientswith complex needsUtiliseyour clinical supervisor/ mentor and other clinical staff appropriately toensure safe and effective patient care, demonstrating awareness of your ownlimitationsProvidesafe, evidence-based, cost-effective, individualised patient care within thesurgery, patients own home or other environment where patient care is carriedoutReferpatients directly to other services/ agencies as appropriate, utilisingPractice and local guidelines and in a timely mannerPro-activelyidentify, diagnose, monitor and manage treatment plans for patients at risk ofdeveloping a long-term condition, as appropriateDiagnoseand manage acute conditions, integrating both drug and non-drug-based treatmentmethods into a management planPrescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national and practiceprotocols, and within own scope of practice.
Utilise Patient Group Directionsfor the administration of medication as appropriate.Workwith patients in order to support compliance with and adherence to prescribedtreatments.Provideinformation and advice on prescribed and over-the-counter medication onmedication regimens, side-effects and interactionsPrioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycareSupportpatients to adopt health promotion strategies that promote healthy living andto apply principles of self-careRecognise,assess and refer patients with mental health needs as appropriateCommunicatewith and support patients receiving bad news Person Specification Knowledge/Skills/ Competencies Essential Awareness of accountability of own role and that required of a Registered Practitioner.
Ability to assess and manage patient risk effectively and safely.
Good communication (oral and written) and inter-personal skills.
Keyboard skills.
Change management skills and ability to support patients to change lifestyle.
Ability to form good working relationships with a multi-disciplinary team.
Ability to listen and empathise.
Understanding of evidence-based practice.
Ability to organise and prioritise workload.
Demonstrable evidence and commitment to professional development.
Qualifications Essential Registered Nurse (NMC) or Registered Paramedic (HCPC) or Registered Physiotherapist (HCPC), Registered Dietitian, Registered Podiatrist or Registered Pharmacist Minor illness qualification at degree level assessed by OSCEs.
Current registration.
Relevant degree.
Independent non-medical prescriber or working towards it, or proven ability to use Patient Group Directions.
Desirable Undertaking a course in Advanced Clinical Practice at Masters level.

Experience of audit.
