Job summary As the largest independent provider of NHS services, Practice Plus Group is delivering a new model of Integrated Urgent Care that reduces pressures on services and staff.
By bringing together NHS 111, clinical assessment and other urgent care services, we enable patients to access the right care, at the right time in the right place.
We are recruiting and looking for ambitious and committed Paramedics and Pharmacists to join our service.
We offer digital consultations.
Youll be working autonomously within your sphere of competency and be part of a friendly, close-knit multi-skilled professional team that includes GPs, ANPs, UCPs, mental health and dental nurses and pharmacy advisors.
You will be involved in providing an enhanced assessment, triage, referral and/or treatment of a wide range of patients, utilising relevant evidence based practice, local guidelines and organisational procedures.
You will provide provision of appropriate care based on patient need, ensuring patients receive a timely and appropriate response.

You will be able to keep your skills up to date with a CPD programme.
You will benefit from regular supervision and education events, including case discussions, learning and clinical updates.
Main duties of the job 1.Facilitate clear patient pathways through primary, community, secondary and tertiary care.
a person centred approach to service delivery, based on clear understanding and experience of the modes for service delivery.
responsible and accountable for assessing health and / or social needs of patients within the PCC setting, using agreed clinical protocols and clinical guidelines software.
able to incorporate the nurses role in health education within the unscheduled care environment.
5.Maintain and develop clinical competence and knowledge of care delivery within PCC setting using both formal and informal training methods.
work closely with the medical practitioners in the provision of quality, evidence-based care to patients within the PCC setting or in patients own home a virtual case load and be accountable for delivery and integrated, quality patient care working autonomously.
take a lead role in the provision of Palliative care and other specialist nursing services within the unscheduled care environment.
provide holistic patient care within the unscheduled care setting from assessment of presenting condition through to diagnosis and initial treatment including the prescription of medication within agreed clinical guidelines and protocols.
hold Nurse prescribing certificate and be competent to prescribe for patients whilst working autonomously.
About us Qualifications Essential: Professional registration with the Health and Care professions Council (HCPC) or GPhC registered Substantial post registration clinical experience in a primary care setting Experience of working in an autonomous advanced role.
Independent Non-Medical prescribing.
Advanced paediatric and/or Minor illness in children experience Experience of senior level decision making and delivery in a changing environment.
Able to understand and analyse complex issues and balance competing priorities in order to make difficult decisions.
Diploma (or equivalent) in primary and Urgent care examination Have completed or currently engaged on MSc level Advanced Clinical Practice education.
Previous experience of triage/telephone consultation Evidence of working across organisational boundaries within health and social care.
Excellent communication and IT skills.
