Job summary At Saint Catherines, our team is dedicated to providing dignified, holistic support for patients with complex health needs at the end of life.
As an ACP, you will be a vital part of our interdisciplinary team, managing personalised care plans, assessing and treating symptoms and disease, and guiding patients and their families through their care journey.
As a qualified ACP you will be expected to contribute to the out of hours medical 1st on call rota.
For candidates joining as trainees, we offer a structured development program with clinical supervision to support you in achieving your ACP qualification.
Main duties of the job To play a crucial role in providing high-quality, patient-centred care to individuals in need of palliative and end-of-life support.
Working as part of the interdisciplinary team, the ACP will assess, plan, implement, and evaluate care for patients with complex needs, helping them to manage symptoms and improve quality of life.
To support the development of an evidence-based practice within the hospice and wider community, in collaboration with healthcare professionals and external partners.
To deliver care in line with the highest professional standards, adhering to the competencies and expectations outlined in the Advanced Clinical Practice in England, and the four pillars of advanced practice.
To complement the medical team and will be expected to contribute to the out of hours 1st on call medical rota.

About the Role Are you an experienced healthcare professional with a commitment to compassionate, patient-centred care?
Do you have advanced clinical skills and a deep understanding of palliative care, or a desire to develop them?
Then join us as an Advanced Clinical Practitioner, a brand-new role within the Hospice.

As an ACP, you will be a vital part of our interdisciplinary team, managing personalised care plans, assessing and treating symptoms and disease, and guiding patients and their families through their care journey.

Key Responsibilities include: Conduct advanced clinical assessments, develop and implement comprehensive care plans, and provide direct patient care to manage symptoms effectively.
Offer compassionate support, guidance, and information to patients and families regarding palliative care options and symptom management.
Collaborate closely with our interdisciplinary team, including physicians, nurses, social workers, and allied health professionals, to ensure holistic, coordinated care.
Contribute to clinical research, audit, and quality improvement initiatives to continuously enhance the care we provide.
About You Essential Qualifications: Registered healthcare professional (Nurse or AHP) with current registration and experience in a relevant clinical setting and a master's degree in Advanced Clinical Practice or willingness to work towards.
Experience: You have experience with complex care management, ideally within palliative, end-of-life care or frailty and demonstrate advanced assessment and clinical reasoning skills.
Attributes: Compassionate, resilient, and adaptable, you're committed to providing dignified, respectful, and empathetic care.
You're a strong team player with excellent communication skills and a genuine commitment to patients and their families.
Knowledge and skills: You are competent within the capabilities for advanced clinical practice in England and the four pillars of advanced practice.
For candidates without the ACP qualification, we welcome applications from those who meet the essential experience requirements and are willing to undertake the masters level training to reach ACP level.

Person Specification Qualifications Skills Knowledge and Experience Essential Registered healthcare professional, RGN or AHP Masters Degree in Advanced Clinical Practice or equivalent experience and qualification or willingness to work towards Significant post-qualification experience in a healthcare setting, ideally in palliative care, community, or acute medicine and frailty Independent Prescribing qualification or willingness to achieve this within 12-18 months Desirable Experience liaising with multiple healthcare providers and coordinating care across different settings (, hospitals, community health, and home care) Participation in clinical audit, education, research projects, or service improvement initiatives aimed at enhancing patient care or clinical practice Previous experience working in a hospice, palliative care, oncology or frailty setting
