Advanced Clinical Practitioner – Canterbury

We are seeking an experienced and highly motivated Advanced Clinical Practitioner with a GPHC (General Pharmaceutical Council) background to join a dynamic healthcare team in Canterbury.
This is an excellent opportunity to work in a forward-thinking, patient-centered environment, where you will play a pivotal role in delivering high-quality clinical services across multiple settings.
Key Responsibilities: Provide advanced clinical care, assessment, and treatment to a diverse patient group in a variety of healthcare settings.
Utilize your expertise to support the clinical team, providing mentorship, guidance, and training to junior staff.
Act as a clinical leader within the team, collaborating with multidisciplinary professionals to ensure the highest standards of patient care.
Conduct comprehensive assessments and develop personalized care plans for patients, with a focus on evidence-based practice.
Manage and review patient progress, ensuring ongoing care delivery is of the highest standard.
Participate in audits, service evaluations, and clinical governance activities to continuously improve service delivery.
Maintain up-to-date knowledge of current clinical practices, guidelines, and regulations.
Develop and maintain strong relationships with patients, caregivers, and other healthcare professionals.
Essential Requirements: GPHC Registration as a pharmacist with post-registration clinical experience.
