Job summary The Virtual Ward environment is the opportunity for patients to experience hospital level care from a multidisciplinary team, in the comfort of their own home.
This is a new clinical pharmacy service and offers you the opportunity to work closely with the team to develop new pathways of care, methods of treatment delivery and experience of medicines governance.The CUH Virtual Wards team are inclusive and the pharmacy post is well integrated into the operations.
This post offers you the opportunities to become a valued member of the team who offers the service oversight of medicines safety and clinical expertise.We are searching for someone with the vision for how the Virtual Ward could develop and offer patients a new and improved experience of care.
Main duties of the job Liaise with the relevant senior pharmacy, medical and nursing staff, other allied health professionals (AHPs) and managers to promote safe, evidence based, safe and cost-effective use of medication in Virtual wards.
Liaise with external health professionals & stakeholders across the Integrated Care System ICS in the management of virtual ward patients GPs, community / primary care network pharmacists, district Nurses.
Provide an advanced clinical pharmacy service to Virtual wards / clinics as part of the MDT.
Provide specialist pharmacist advice and services to the Virtual wards, the specialist MDT and patients under their care.
Prescribe medication for individual patients within the agreed scope of practice and according to the CUH pharmacist prescribing policy; Participate in and lead where required on novel pharmacy projects in the care of Virtual wards patients including pharmacist participation in outpatient clinics and optimizing the management of Virtual wards patients.
Identify and undertake research to inform practice developments and underpin delivery of an effective, evidence-based pharmacy service.
Provide pharmacy leadership to the Virtual wards.
To support the Trust with medication / pharmacy related aspects of patient flow.
Provide pharmacy leadership to the virtual wards.
To ensure adequate rostering and service level plans for pharmacy virtual wards are in place, including plans for Pharmacy support over weekends, bank holidays & out of hours.

Person Specification Qualifications Essential Masters level degree in pharmacy.
Registered as a pharmacist with General Pharmaceutical Council (GPhC).
Diploma in Clinical Pharmacy/General Pharmacy Practice (or equivalent) Desirable MSc in Clinical Pharmacy (or equivalent)*.
Management qualification.
Registered as an independent prescriber with the GPhC or commitment to complete within 6 months of appointment Member of Royal Pharmaceutical Society.
Member of the faculty of the Royal Pharmaceutical Society (or working towards) at Advanced Stage II.
Experience Essential significant post-registration experience including prescription screening, medicines reconciliation .
significant experience of patient consultation.
3.Experienced clinical pharmacist at band 7 or band 8a who has covered a range of specialities including the designated specialist clinical area.
4.Experience of documenting pharmaceutical care issues in medical notes.
5.Experience of writing professional reports.
6.Experience of writing drug policies and treatment guidelines.
7.Experience of developing and reviewing patient group directions and shared care guidelines.
8.Evidence of having conducted and implemented the outcomes of a clinical audit.
9.Experience of the practical application of antimicrobial stewardship initiatives.
10.Experience of the management of high-cost and high-risk medicines.
11.Experience as a personal tutor to a range of staff groups.
12.Experience of developing and leading on projects including change management and service development.
Desirable Recent significant post-registration hospital experience, ideally including previous experience in a teaching hospital and experience of more than one hospital.
Recent practical experience of independent prescribing for a designated group of patients.
Experienced clinical pharmacist (at band 8a or 8b) including one or more of the following: surgery, neurosciences, critical care, transplant, medicines information, acute and emergence medicine.
Evidence of having written pharmaceutical and/or educational publications.
Experience of drug expenditure analysis and reporting.
Experience of horizon scanning.
