Job summary We are looking for an Advanced Clinical/Nurse Practitioner to join Leeds City Medical Practice on either a full or part time basis.
The post holder will be able to demonstrate safe, competent clinical decision-making and deliver expert care, including assessment, diagnostic skills, treatment and evaluation.
As well as taking on responsibility for the management of the acute medication requests for the practice.
Main duties of the job Work collaboratively with the whole practice team to deliver an excellent service that meets the medical needs of patients.
Consult with patients taking full history, assess, diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods and agree management plans.
Work autonomously at a level appropriate to clinical competence and within the scope of professional practice following local and national protocols.
Demonstrate a high level of clinical decision-making skills and provide a range of clinical care, underpinned by NICE Guidelines and evidence-based theory.
Prescribe in line with current guidelines and review and monitor patient medications.
Maintain clinical competence in a range of extended skills as required to meet the needs of the patient.
Ensure appropriate, timely and efficient referral to other health and social care bodies.
Support the rest of the team in developing and promoting high quality service based on best practice and underpinned with a sound theoretical knowledge.
Participate and contribute to all clinical targets (QOF, QIS, ES etc) and infection control across the practice.
Maintain patient confidentiality at all times and be alert to Safeguarding issues or concerns.
Visit patients who are housebound in order to carry out clinical assessment when required.
Adhere to Professional Code of Conduct at all times.

We are a large busy Training Practice with ten GP Partners across two sites in Beeston delivering care and services to a population of over 19,400 patients.The role requires resilience and the ability to be flexible and adaptable, confident to work independently undertaking autonomous decisions made in practice.
