This is an exciting opportunity for a Advanced Practitioner who is dedicated to delivering high-quality care, withhighly developed triaging skills that thrives in a dynamic healthcare setting.
Main duties of the job The post holder will be a highly qualified and experienced Registered Nurse or Paramedic who will be responsible for working autonomously within primary care settings including home visits providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care working within their scope of advanced clinical practice.
The post holder will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients presenting within primary care with health needs to ensure every patient receives high quality clinical care.
The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process.
They will communicate and work collaboratively with the general practice to meet the needs of patients.
They will also be responsible for leading clinical teams and for the development and consistent delivery of policy, procedures and using critical thinking, decision making and risk analysis skills to identify and take forward service innovations.
