Job summary Whittle Surgery is looking for an Advanced Care Practitioner to work 24 hours a week Wednesday to Friday as part of a large multi-disciplinary team.You will join a friendly and helpful team who strive to provide excellent care to our patients.
Main duties of the job The role involves seeing patients in practice, carrying out triage and visiting patients in their own home/care setting.You will also be responsible for administrative work such as reviewing hospital letters and investigation results on a daily basis.
About us Our team includes 6 GPs, 2 advanced nurse practitioners, 1 first contact practitioner, 1 pharmacist, 1 pharmacy technician, 1 diabetic specialist nurse, 2 practice nurses, 2 student nurse associates and 1 health care assistant.
We also have a large administrative team to support our clinical team.We have 11,500 and have recently moved to a total triage system.
We look after 3 care homes and patients in supported living.We are a supportive team who work hard to look after our patient population.
Job description Job responsibilities Clinical PracticeThe Advanced Care Practitioner works autonomouslyand is accountable for own professional actions.Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.Assess, diagnose and treat patients attending surgery or via home visits with a range of acute, non-acute and chronic medical conditions.Assess, diagnose and treat patients in surgery or own home who require acute medical attention and refer to appropriate agency.Directly admit patients to secondary care hospital in acute medical need.Refer patients to other medical specialties for assessment if deemed appropriate.Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations.Undertake and interpret a range of diagnostic tests and routine clinical procedures venepuncture, blood pressure monitoring etc.Make critical judgements of the highest order to satisfy the expectations and demands of the job first contact with patients presenting acutely and manages care in the presence and absence of protocols.Make decisions where precedents do not exist, where appropriate without recourse to others direct referrals of patients to hospital consultants without prior reference to other medical colleagues.Advise and support others where standard protocols do not apply.Work within professional guidelines and codes of conduct.Work within all relevant practice policies and procedural guidelines infection control, chaperoning and risk management.Extended and supplementary prescribingCare and Programme Management Develops and manages highly specialised programmesof care and care packages and provides highly specialised advice concerningcare normally beyond the scope of normal working practice establishesdifferential diagnoses, management/treatment plans and makes referral asappropriate.
