per hourWhat will your responsibilities be?In this role you will undertake reviews of care packages, contribute to effective Multi-Disciplinary Team working by participating in meetings and collaborating with others to achieve the best outcomes for those accessing services and complete Continuing Health Care (CHC) checklists and identify those that require full assessments.
