Connect2Halton are looking for a Social Worker in the Intermediate Care and Frailty Service team.
You will provide essential support to individuals with complex needs, focusing on prevention, recovery, and facilitating safe discharge from hospital settings.
Hourly rate: from £24.71ph (depending on experience) This role involves working collaboratively with multidisciplinary teams to assess and coordinate care for elderly and frail clients, helping them maintain independence and well-being.
Key Responsibilities Conduct comprehensive assessments of clients, identifying their physical, mental, and social care needs.
Develop and implement personalised care plans in collaboration with the client, their family, and other healthcare professionals.
Coordinate with various professionals, including physiotherapists, occupational therapists, and doctors, to support client recovery and prevent hospital readmissions.
Advocate for clients, ensuring their voice and preferences are central to care decisions.
Support the discharge planning process from hospitals to community or home-based settings.
Monitor and evaluate care plans regularly, adapting them based on client progress and needs.
Provide crisis intervention support when required, especially in cases of unexpected deterioration.
Maintain accurate records, case notes, and reports in compliance with relevant policies and procedures.
Requirements A valid social work qualification and registration with Social Work England.
Demonstrable experience working in adult social care, ideally in intermediate care or with frail and elderly populations.
Strong understanding of social work principles, legislation, and best practices in safeguarding vulnerable adults.
Excellent communication and interpersonal skills, with the ability to collaborate effectively in a multidisciplinary setting.
Ability to assess and manage risks, make complex decisions, and prioritise tasks effectively.
