Responsibilities include: Conduct thorough assessments of adults' needs in line with the Care Act 2014 and relevant legislation.Develop and implement individualized care plans to foster independence and support community integration.Safeguard vulnerable adults by providing targeted interventions to ensure their safety and well-being.Work collaboratively with multidisciplinary teams, families, and community services to provide comprehensive support.Advocate for adults' rights, ensuring access to necessary resources, services, and benefits.Regularly review and adjust care plans to respond to evolving needs and situations.Uphold statutory responsibilities and follow best practices in safeguarding and care provision.
