Main duties of the job Thepost holder will be an excellent communicator and work well both individuallyand within a team.
Theywill carry out planned and unplanned health (nursing) interventions within thecontinence service.They will be required to undertakeholistic continence assessments, and work with patients to developindividualised treatment programmes to promote continence.
The post holder will hold clinics inboth Sunderland and South Tyneside and visit patients in their home when thepatient is unable to attend clinic.
They will be a lead nurse for identifiedcare homes.
They will contribute to the delivery ofthe education programme for both Trust Staff and care home staff.Informal visits encouraged.

Degree in a nursing/health related subject or working towards.
