Job summary A patient's recovery can be impacted by staying in hospital longer than is necessary so having the right care plan in place to support the person to leave hospital at the earliest and safest time for them is crucial.The Acute Trust Case Managers support the effective management of patient discharge from hospital and play a pivotal role in supporting ward-based colleagues with patient discharge plans.The Acute Trust Case Managers work with all members of the newly formed Transfer of Care Hub (ToCH) based at the Weston General Hospital , including social workers, Sirona CToCH (Community Transfer of Care Hub) team and community therapists and will be encouraged to participate and share learning with colleagues across the team.We are looking for Registered Nurses or AHPs (Allied Health Professionals) to join our team.
Main duties of the job Use your expert knowledge to coordinate, facilitate and drive all patient discharges on the wards.
Be responsible for a defined caseload working with specific wards on a rota basis Manage complex discharges, supporting /completing initial assessments for Continuing Health Care (CHC) fast-track assessments, and referrals to Community Partners (Sirona) for pathway beds.
Liaise with the wider MDT, local authorities (LA) and ICB to drive discharge at all levels.
Ensure that National policies and guidelines are adhered to.
Ensure that patients and relatives are at the heart of everything we do involving, supporting and communicating with them throughout the discharge process.
Build key relationships on the wards with all staff to ensure collaborative working relationships are cultivated to support effective and timely patient discharge.
Provide line management for patient flow coordinators and discharge flow coordinators.

Person Specification Qualifications and Training Essential Registered Nurse or HCPC registered Evidence of recent CPD Knowledge and experince Essential Demonstrate a broad post registration experience In-depth experience of working within a multidisciplinary team in an acute Trust Experience of discharge planning of complex patients Evidence of specialist knowledge relating to patient discharge Knowledge of relevant legislation relating to patient discharge Desirable Experience of working with, or in Community healthcare Experience of Line Management Experience of undertaking projects/audits(D) Skills and abilities Essential Demonstrate excellent clinical assessment, planning, delivering and evaluation skills Ability to manage complex patient cases Ability to motivate and influence staff Remain professional and focussed during pressurised situations Ability to organise and prioritise a large and varied workload Excellent written, verbal and electronic communication skills Ability to use IT systems including Careflow EPR, databases, Careflow workspace and other clinical and non-clinical systems required to perform the job role Ability to lead meetings Ability to manage own caseload with minimal supervision Ability to follow complex instructions and protocols accurately in short timescales Ability to work independently Ability to review & reflect on own skills & knowledge, and to lead own CPD in conjunction with supervisors & peers Desirable Ability to interpret and relay patient discharge data Aptitudes Essential Enthusiastic and self-motivated Innovative and participative in policy change Team Player Can do attitude Ability to communicate effectively & sensitively with a wide range of people Ability to manage own time appropriately & to prioritise tasks Excellent organisational skills Ability to meet deadlines & work under pressure
