An opportunity has arisen within NHS South Central and West Commissioning Support Unit (SCWCSU) to join the Continuing Healthcare Care (CHC) team as Continuing Health Care (CHC) Nurse Assessor, to deliver an effective assessment, review, and case management service.
We are looking for candidates with professional registration from a range of backgrounds who can evidence how their qualifications, skills and experience will contribute to SCWCSU values and strategic goals.

Candidates must be able to demonstrate knowledge and experience of the CHC legislation and process and have experience of working within the field of CHC.
Person centred assessment, including risk assessment, and case management skills are essential.
Main duties of the job Undertaking Continuing Healthcare (CHC) assessments/ Decision Support Tool (DST), Care needs assessments, Personal Health Budget Care and Support Plan and reviews for patients across all aspects of their care.
Chair multi-disciplinary team meeting when completing DST ensuring that information on DST is evidence based.
Produce quality DSTs before verification.
If DST is completed in the community, complete care needs assessment.
Checklist and fast track screening/ triage.
Mental capacity assessment and Best Interest Assessment.
Manage safeguarding referrals for all individuals fully funded via CHC.
