Physiotherapy and Occupational Therapy Department Band 4 Therapy Assistant Practitioner 24 hours / permanent We are looking for a highly motivated and enthusiastic candidate with relevant experience within Occupational or Physiotherapy As a Therapy assistant practitioner you will be allocated a delegated patient caseload by the qualified physiotherapist and Occupational Therapist.
This role is primarily in place to help support discharge of patients from hospital from wards across the whole site.
You will contribute to discharge planning by carrying out environment visits to assess suitability of the patient's home for various pieces of manual handling and care equipment.
In addition and when relevant you will be expected to work independently, delivering therapy interventions on the wards by assessing the patient's needs, modifying treatment and care plans, and delivering this care without supervision.
This will include progressing patient function, reassessing and discharging within scope of practice.
Full engagement with the multidisciplinary team is another key part of this position.
Excellent communication skills are essential You will also support the Therapy teams through a range of non-clinical duties, from equipment ordering, referring patients to primary care teams, to assisting with statistical information collation for patient activity audit purposes, as well as make telephone calls to carers and care providers.
The range of conditions encountered and requirement for excellence in clinical reasoning and team working in this post will allow the successful candidate to gain extensive and in-depth experience as a band 4 Therapy Assistant Practitioner.
Therapy Services support staff development through appraisal processes, supervision and CPD opportunities.
Candidates must display evidence of clinical experience of work in a variety of areas ideally including Medicine and Medicine of the Elderly, and will have relevant / current experience as a band 3 therapy support worker, or as a band 4 assistant practitioner.
