Job summary This is an exciting opportunityto work in a brand-new role within Pershore & Upton Primary Care Network (PCN).The role will give you a unique chance to work as a member of Pershore &Upton PCN team, whilst also building links with a range of other local servicesand be part of a county wide Work and Health coach network.
We willparticularly welcome applicants with lived experience and those with strongconnections to the Pershore/Upton area.The successful candidate willbe part of the Workwell programme and will work with people from a range ofbackgrounds to help them return to work.
They will work closely withparticipants within the local community to assess the barriers they experience inreturning to work and provide support and coaching along with health andemployment advice to overcome these barriers and return to work.This role would suit someonewith previous experience of coaching and mentoring, clinical experience is not required,and training will be provided for the successful candidate.Workwell is aprevention and early intervention service to support people to overcome healthconditions that affect their ability to start, stay and succeed in work.
The post holder will be one of about 15coaches that works in Primary Care, either in a PCN or a GP Federation acrossHerefordshire and Worcestershire.
Main duties of the job The core purpose of the role is tosupport participants to Start, Stay and Succeed in work.
The post holder will be expected to workflexibly, using their judgement, training and professional experience toachieve this goal.
This may include thefollowing non-exhaustive list of activities:1.Receiveand process referrals directly from Primary Care colleagues with the PCN thepost holder is based in2.Receivereferrals from Job Centre Plus and Employers3.Validateparticipants eligibility prior to offering support.
4.Managea caseload of participants who have common mental, physical and social healthproblems or long-term conditions.
5.Carryout assessments to identify barriers and develop action plans in agreement withthe individual to overcome those barriers 6.Providecoaching and other light-touch holistic support on a 1:1 basis to support thedelivery of the participants plan, 7.Onwardreferral to other services where additional support beyond the 1:1 support isrequired.
This may include a wide rangeof health and employment related support as needed to address the participantsneeds.8.Workwith external organisations as needed to support the participants journeythrough the Workwell pathway.9.Delivergroup sessions for participants where cohorts exist and enable a more efficientway of working.Please see the attached job description for further information.

Job description Job responsibilities JobContextThis is a WorkWellemployment support project for eligible adults in Herefordshire andWorcestershire, where the successful candidate will assist people within thePershore and Upton PCN catchment area.
Broad scope of service: Participantshome address OR address of their GP and local Jobcentre Plus must fall withinHerefordshire or Worcestershire.
Participantsmust be over 16.Participantsmust have the right to work in the UK.
Participantshave a health-related barrier to work.
The servicewill assist participants to progress their journey towards employment who areexperiencing barriers into employment, support participants to remain in workwho are on long term sick leave or struggling in the workplace due to a healthcondition.
The service willuse a coaching based approach and use light-touch holistic therapy or triage toprovide early-intervention work and a health assessment service.
The servicewill provide advice and training to potential employers on reasonableadjustments and retention of employees where necessary.
This service willreduce health inequalities as employment is evidenced as a way of maintaininggood health and well-being.
Flexibility andadaptability are required due to the unpredictability of the environment andresource availability under which work will need to be carried out.
Staff will workto Vertis Healths lone worker policy and ensure that they have a mobile phonein working order so that communications can be achieved at all times.

To maintain detailed and accurate casenotes, action plans and WorkWell participant paperwork and upload these on tothe management information system within the allocated timeframe.11.
To collect and record specifiedmanagement information, as determined by the line manager, and respond torequests for management information from service commissioners and DWP.12.
Provide mutual aid to other PCNs or Workand Health Employment Coach when other areas require support, subject tocapacity of the post holder (as assessed by their line manager).13.
Support training with colleagues fromexternal organisations to help raise awareness and improve their knowledgeabout this pilot and the WorkWell model, as well as supporting them tounderstand the common barriers and Long-Term Conditions.

To travel across a reasonable area (asdetermined by the line manager) to deliver services to participants who areunable to access the service at the normal base.
