Job summary To lead in the delivery of high standards of Physiotherapy/Occupational therapist interventions to patients within their own place of residence to prevent avoidable admissions to acute care and to support discharges.
Post holders will provide short term reablement/ rehabilitation programmes with effective short term goal setting.
This will involve using health coaching skills to effectively engage with the patient and their family to ensure expectations are appropriately managed.
The role requires flexibility, working across the following services; NEAT, Discharge to Access and Virtual ward to form part of the Urgent Care Response Team.
