Job summary This post will be part of the therapy team supporting patients who attend services delivered at Priscilla Bacon Lodge.
It is a new role and a very exciting development.
The Assistant Practitioner will function at a higher competency level than that of other health care support workers.
The post holderwill be expected to contribute to the delivery of high standards of care and service delivery to inpatients and patients who attend day and outpatient services This role will have a responsibility for actively participating as a key member of the SPC Therapy Team and will be expected to communicate with a range of people on a range of matters (including the wider multi-disciplinary team) within the clinical department and/or ward, community setting and acute settings.
The assistant practitioner will have a responsibility to provide quality services to patients under the supervision of a registered Therapist.
The post holder will be expected to support care planned with the therapist and independently: review and progress care in a supportive way.
Main duties of the job This role encompasses clinical and non clinical duties.It is an opportunity to develop clinical skills to assess, plan, deliver and review care with the support of a registered therapist to ensure patients reach their full potential - such as for rehabilitation or to reach their preferred place of care.The post holder will work with the wider MDT and will work across inpatient and out patient settings delivering bespoke care to the specialist palliative care patient group whilst offering support and education to both patients and their familiesCommunicating with patients, their families, staff, and outside organisations is a vital part of the roleIT literacy is important to ensure patient records are kept up to date and that equipment needed can be ordered in a timely and efficient wayThere will be a need to attend meetings and patient review meetings to represent the therapy aspect of care.The post holder will be able to report risks, incidents and support colleagues to reduce these to ensure patient, visitor and staff safety at all times About us Find out more about working for our organisation here: Job description Job responsibilities The role encompasses clinical and non clinical aspects including.
Supporting in patients with discharge planning and rehabilitation through assessment and executing therapy-based interventions with supervision from a registered therapist Building strong working relationships with patients, their families and colleagues to foster an environment conducive to empowering the patient to reach their full potential.
Supporting the delivery and preparation of therapeutic groups Demonstrate problem solving, and contribute to the solution, working with colleagues to this end.
Ensure patient held records are completed for each patient and that SystmOne inputting is completed concisely, accurately, and timely in line with service standards.
Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
To participate in multi-disciplinary/multi-agency meetings as appropriate.
Provide basic equipment and be responsible for ensuring that it is used safely, following appropriate training, and that it is maintained.
Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
Demonstrate an awareness and understanding of consent, and gain consent as appropriate following department policy.
Share responsibility for maintaining store cupboards, cleaning specialist equipment and general housekeeping and administration tasks.
Share responsibility for indirect patient contact tasks, such as, answering telephones, arranging appointments, processing referrals and inputting activity data.
Demonstrate basic IT and standard keyboard skills.
Be fully aware of learning opportunities and participate as appropriate and ensure all mandatory training is up to date.
To adhere to the Behaviour Framework Participate in Risk assessment and completion of Datixs.
Support all colleagues.
Always employ sensitive communication skills.
Ensure method of communication is appropriate at all times.
Participate in supervision as per Trust policy Person Specification Qualifications Essential oFoundation degree level knowledge and experience or the equivalent .
Experience of working in a health or social care setting Desirable oDemonstrable knowledge and skills acquired through training and experience.
Experience Essential oExperience of working with palliative care patients oEvidence of recent mandatory training oInvolvement in care planning Desirable oNHS experience oPrevious experience of being a generic therapy worker within a multi-disciplinary team.
oExperience of supporting group work Skills, Abilities and Knowledge Essential oAbility to use own initiative oGood communication, observational and organisational skills oThe ability to: Manoeuvre patients using handling aids.
IT knowledge Observation and reporting skills Evidence of own development through PDP Understanding of the needs of patients undergoing palliative rehabilitation Recognise own limitation and reports as appropriate Leadership skills A team player Highly motivated Desirable o2 Years' experience at least one of which relevant to the post.
