Job description Job responsibilities Management ofChronic Diseases:Identifyingsignificant abnormalities using a holistic approach to history taking andobservation and assessment of physical systemsDiagnosis,monitoring and development of individual management plans; agreeing these asappropriate with the patient and other health professionalsUnderstand theaims of management and the pharmacological treatments for chronic diseasesInitiate, performand interpret diagnostic procedures as appropriate for chronic diseasemanagementSupport recallprocedures, appropriate literature and health promotion, and auditMaximise QualityOutcomes Framework incomeManagement ofMedicines:Diagnose and manage both acute and chronic conditions, integrating bothdrug-and nondrug-based treatment methods into a management plan.Review medication fortherapeutic effectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice.Work with patients in order to support compliance with, and adherenceto, prescribed treatments.Provide information and advice on prescribed or over-the-countermedication, medication regimes, side-effects and interactionsManagement andAdministration:Liaison withother Nursing staff, Doctors, District Nurses and other agencies within thehospital and social servicesTreatment Room /Health Promotion and Screening:Undertake routinetreatment room procedures for this cohort of patients as and when required,including but not limited to immunisations and recording of vital signs asnecessary.
