Job summary An excellent opportunity has arisen to join our team as a UCR and Intermediate Care Specialist Occupational Therapist based in Scarborough.
This is a 6 months fixed term post.We are looking for an experienced Occupational Therapist, to work with our urgent community response and intermediate care team providing clinical input in order to facilitate timely discharge from an acute hospital stay or to remain safely at home when facing an unexpected health crisis.
The role also includes bed based intermediate care serviceThe role includes integrated MDT working targeted at people living with frailty, multi-morbidity and/or complex needs to help them stay independent and healthy for as long as possible.

As a senior therapist you will have management and supervision responsibilities for junior staff & students.Part time / flexible working options will be considered for this post and can be discussed if required.
Main duties of the job The main duties of the role include: To provide high quality and timely clinical triage for referrals coming into the service, providing advice and guidance to referrers and liaising with internal teams and partner organisations to support patient care deliveryTo be an integral part of an MDT consisting of Nurses, Physiotherapists, Occupational therapists and Support workers delivering patient centred care to maximise independence, confidence and ability.To demonstrate specialist clinical skills in practice for patients in need of UCR or Intermediate Care supportSupport patients with individualised goal setting, education and care plans to work towards fulfilling their potential in a targeted and realistic way via delivery of evidence based clinical interventions and practice in partnership with patients, relatives, staff, managers, and externally with other agencies.To provide support/advice regarding service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies.To provide line management, supervision, support, guidance and training to peers, Community Support Practitioners and students.

Person Specification Qualifications and Knowledge Essential Current HCPC professional registration Professional knowledge acquired through degree or equivalent, plus short specialist courses, experience to post-graduate equivalent diploma level Diploma or Degree in Occupational Therapy Desirable Leadership qualification/equivalent experience if required Previous experience working as a community based Occupational Therapist Evidence of continuing learning and CPD Experience Essential Evidence of continuing professional development (CPD) relevant to the clinical area at a specialist level of care Experience of case load management within specialist service area, some of which may be complex Evidence of promoting/supporting active patient/relative involvement/participation Desirable Working knowledge of Trust policies and procedures.
Skills and Competencies Essential Effective inter-personal skills and experience in supervising and fieldwork education/mentorship Able to demonstrate effective communication skills and work flexibly within the team/service Good time management and organisational skills Ability to motivate staff to embrace change Desirable Ability to manage daily operations and maintain a safe working environment
