To offer support to existing social workers and to support the development of Social Work apprentices andstudents.The socialwork lead should demonstrate passion, professional autonomy and an ability towork across organisations in ensuring the best outcomes for individuals who useour service.
Main duties of the job As a Social Work Lead, youwill:Manage and take alead role in the professional development of Social Workers throughsupervision, mentoring, CPD activities and offering advice.You will be expectedto promote a culture of professional curiosity, encouraging accountability androle modelling this in your own best practice.
Embed the North EastLincolnshire Practice framework within practice You must be able todemonstrate your practice, experience and knowledge of legislation and be focusedon delivering positive outcomes for the local population.You will lead onensuring Navigo is compliant in the delegated responsibilities under Section 75partnership agreements, legally provided by the NHS Act 2006, this includes theapplication of the Care Act (2014) and responsibilities associated with S117 aftercare.You will line managethe specialist Social Work posts attached to CMHT.

Job description Job responsibilities Communicates highly complexand highly sensitive information to Service Users during the course oftreatment where there may be barriers to understanding and a need to achievepositive engagement.Shares information relating toassessment and care plans with Service Users, Carers and families in aresponsive, empathic manner, taking into consideration the possiblecomplexities of the familys circumstances and the highly sensitive nature ofthe informationCommunicates effectively withall professionals involved in the Service Users care including all members ofthe Multi-disciplinary Team, General Practitioners, Local Authority, Educationproviders, Voluntary agencies, User/Carer groups etc, to maximize the qualityof service delivered to Service Users and their families and carers.Develop relationships withproviders across PLACE such as FOCUS, NELC, Public Health, ICB, CPG and thevoluntary sector Arrange student social workplacements/apprenticeships partner in the Place Training academy, ASYE leadand moderatorWork alongside the PSW forNorth East Lincolnshire on specific projects involving navigo and promotingsocial work across PLACE Analytical and JudgementalSkillsAnalyses highly complexService User information, derived from a variety of sources which may conflictor be incomplete, using assessment tools to develop care plans.Evaluates and makes decisionsabout support options, taking into account both theoretical and therapeuticmodels and highly complex factors that have shaped the individual, family orgroup.
Makes decisions around positiverisk taking and decisions to discharge.Responsible for recognisingthe potential for or signs of Service User harm, abuse or neglect, includingpoor clinical practice, reporting all such concerns and taking all reasonablesteps to protect the Service User.Responsible for identifying and reporting concerns regarding thesafeguarding of children who may be at risk.Support the Social Workers ofNavigo in embedding the PCF framework into their practice, offering supervisionand CPD sessionsPlanning and OrganisationalSkillsProvides expert advice,guidance and consultation to other staff contributing directly to Service Usersdiagnosis, formulation and intervention plan.Facilitates decision-making byplanning and mapping processes/ interventions with members of the team or thewider nursing service to determine the most appropriate treatment modality orservice for Service Users.Leads the multi-disciplinarycare team in the development and implementation of multi-disciplinary careplans where appropriate.
Monitorsprogress during the course of multi-disciplinary interventions.Provides expertise and adviceto facilitate the effective and appropriate provision of care by all members ofthe wider team across the designated service and locality.Responsibility for ServiceUser/Service User Care, Treatment and TherapyAccountable for ownprofessional practice in the delivery of highly specialist care to ServiceUsers with severe and enduring mental health conditions or learningdisabilities which include psychiatric, physical and social co-morbidity andsignificant behaviours that challenge.Plans individual Service Usercare and where appropriate co-ordinates care within the Multi-Disciplinary Team(MDT) and external agencies.Responsibility for Policy andService Development ImplementationThe Social Work Lead isresponsible for identifying service gaps and develop plans and programmes toaddress identified needs.The post holder will play akey role in supporting the clinical leadership team in the overall operationalplanning and development of the specified service area advising seniorcolleagues on those aspects of the service which may need addressing at a broaderstrategic levelTake a lead in strategicservice development projects as required ranging from acting as a resource insetting a strategic direction for the specified aspect of service to thedevelopment, agreement and management of specific project plans in line with agreedNavigo/locality business objectivesPlay a key role in theprofessional development of healthcare professionals through integration intoappropriate academic and research committees locally and nationally.
Identify specialist training needs for professionalsDevelop and deliverevidence-based training and development programmes to nursing andmulti-disciplinary colleaguesResponsible for identifyingand proposing changes to policies and procedures which may impact onmulti-disciplinary teams across the locality.Including the development of policy and procedure to ensure sound governancecommensurate with practice guidelines.Responsible for robust implementation of policies within own area ofresponsibilityLead in the demonstrablemeasurement of clinical effectiveness of any change process in line with therequirements of clinical governance Person Specification Knowledge Essential Substantial evidence- based practice relevant to the role Advanced, in-depth knowledge of audit and development and quality improvement methodologies and their application.
Ability to assess all levels of risk within an evidence-based framework.
In-depth applied knowledge of statutory legal frameworks Clinical governance and its application in practice Applied knowledge of relevant guidance , NICE Principles of managing change.
Social Inclusion agenda Quality improvement methodology Desirable Awareness of how to undertake mental health assessments incorporating biological, psychological, cultural and social perspectives Experience Essential Liaison and joint working with statutory, voluntary and private sector agencies.
Working with Service Users as covered by the Mental Health Act, Mental Capacity Act, MHA Code of Practice, Care Act Providing clinical and professional leadership to other qualified and non-qualified members of the profession.
Managing multi-agency service delivery models.
Autonomous clinical decision making Developing and delivering training programmes and clinical supervision Project management of quality and service improvement initiatives.
Experience of managing change implementing new models of care, new pathways, service development activities Goal setting and delivery of targets within deadlines.
Working well under pressure, within tight deadlines, and producing good quality results Leading formulation, review and treatment within the context of the multidisciplinary team Assessing capacity to consent to treatment.
Qualifications Essential Professionally qualified registered Social Worker registered with Social Work England.
Minimum of 5 years post professional qualifying clinical experience Band 6 and above Evidence of postgraduate study - certificate or diploma or Masters within the candidates professional field.
Be prepared to undertake a specific qualification where the role is required.

Compile and complete statutory documentation Demonstrated ability to present evidence to courts, tribunals and managers hearings.
Communicates in a way which recognises difference and ensures that people feel included and their individual communication needs are met Analytical & Judgemental Skills Provide effective clinical leadership skills that are capable of establishing direction and purpose and leading others Adapt creatively the evidence base for interventions in mental health for Service Users with communication difficulties / learning disabilities.
Provide consultation to other professionals and non-professional groups Undertake complex multiagency working and liaise with multiple systems Identify, provide and promote appropriate interventions / means of support for carers and staff exposed to highly distressing situations and behaviours that challenge Utilise appropriate clinical governance mechanisms within own work.
Planning & Organisational Skills Able to work flexibly and co-operatively as part of a team Able to use own initiative and make decisions independently Use multimedia materials for presentations in professional, academic and public settings IT literate standard keyboard skills Time management skills Teaching and presentation skills Influencing skills Analytical and problem solving skills Team-work skills.
