Job Details: Job Reference : HCC619100 Salary Range : £32,149 to £35,241 per annum   Work Location : Countywide, Hampshire (Hybrid Working) Hours per week : 37 (flexible working hours considered) Contract Type : Permanent Closing Date : 1 December 2024 Interview Date : w/c 9 December 2024   As a Senior Case Worker in the Hampshire County Council Continuing Healthcare (CHC) team, you'll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC.
You'll be supported to manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities.
What you'll be doing :   Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
Undertaking assessments in line with the Care Act 2014.
Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
Collaborating with individuals and their families to transition them to suitable care provision following assessment.
What we're looking for :   Knowledge of Care Act 2014 and it's eligibility criteria.
Knowledge and / or experience of National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022 (Revised).
Good verbal and written communication skills.
The ability to work under pressure in a busy working environment.
Strong analytical and problem-solving skills.
Organisational and time management skills.

This role requires travel across Hampshire and working 20% of your week in office.
