Job type Fixed term contract Location Birmingham B30 3AS Full job description Main Duties & Responsibilities Clinical Responsibilities In accordance with the Practice timetable, the post-holder will undertake a variety of duties including surgery consultations, telephone consultations, checking and signing repeat prescriptions, and dealing with queries, paperwork, reports, and correspondence in a timely fashion.
Triage patients presenting to the surgery by telephone, online, and in person.
Make professional, autonomous decisions regarding presenting problems, whether self-referred or referred from other health care workers.
Assess the health care needs of patients with undifferentiated and undiagnosed problems.
Screen patients for disease risk factors and early signs of illness.
In consultation with patients, develop care plans for health and promote healthy lifestyle strategies and education.
Manage long-term conditions and patients with complex problems.
Record clear and contemporaneous consultation notes to agreed standards.
Collect data for audit purposes and comply with QOF requirements.
Deliver enhanced services.
Check, manage & process patient test results.
Refer patients to secondary care and/or relevant organisations as required.
Compile and issue computer-generated acute and repeat prescriptions in line with local and national guidelines.
Prescribe in accordance with the Practice prescribing formulary whenever clinically appropriate.
Other Responsibilities within the Organisation Awareness of and compliance with all relevant Practice policies/guidelines.
A commitment to life-long learning and audit to ensure evidence-based best practice.
Contribute to evaluation/audit and clinical standard setting within the organisation.
Contribute to the summarising of patient records and read-coding patient data.
Attend training and events organised by the Practice or other agencies, where appropriate.
Keep up to date with current evidenced-based approaches to patient care and service delivery.
Contribute to education and training of trainee members of staff and students.
Attend and contribute to practice and PCN meetings.
