Job summary We have an exciting opportunity for a Band 4/5 Pharmacy Technician to join our experienced Medicines Management team at Hampshire and Isle of Wight Healthcare NHS Foundation Trust.We are looking for a highly motivated, enthusiastic, reliable Pharmacy Technician to be involved in delivering enhanced medicines management service on a rotational basis across Basingstoke and North Hampshire Hospital and Parklands Hospital.
Promoting the safe and effective utilisation of medicines in all care situations, working with care staff and patients directly to ensure patients receive the highest standards of care in the management of their medication.
Duties will include medicines reconciliation, assessment of patients own drugs (POD's), checking of medication for discharge and the counselling of patients.You will be managed by the Lead Medicines Management Technicians and Education and Training team to ensure you are following the legal, ethical, professional and employers code of conduct.
Main duties of the job To identify patients newly admitted to in-patient units to obtain an accurate medication history and ensure timely medicines reconciliation to facilitate a smooth admission service.Appropriately referring interventions back to the clinical pharmacists or liaising with other members of the MDT within scope and experience.Documenting interventions and discrepancies on the electronic patient records.To facilitate the supply in a timely fashion of any non-stock medication for individual patients with insufficient medication for their current treatment.To assess the suitability of any Patients Own Drugs for use.Dispensing inpatient, outpatient or discharge medication as required.To facilitate safe discharge by organising timely supply of new or additional items required for discharge where insufficient is held on the ward.To counsel patients, their carer's, and healthcare professional on doses, frequency, side effects, benefits of taking medicines/completing courses of medication, interactions with other medicines, using tact and overcoming barriers to understanding.To dispense prescriptions from outpatients, wards and other clinical areas, ensuring that standard operating procedures are followed.Handing out the final dispensed product including counselling for patients, carers or healthcare professionals as required.Performing locker checks for patients who has been an in-patient for longer than 7 days.
