Job summary Are you looking to be part of a compassionate, innovative team dedicated to providing exceptional respiratory care?We are looking for a Specialist Nurse or Physiotherapist to support our Respiratory Admission Avoidance for patients that present with Acute Respiratory Infections.
You will work closely with Portsmouth Hospitals University NHS Trust, primary care, and other community care services to facilitate safe and effective care for patients.The team provides urgent care to people in their homes which helps to avoid hospital admissions, supports earlier discharges from hospital and enables people to live independently for longer.We also provide virtual wards which enable a safe alternative to hospital for patients living with long term respiratory conditions, allowing them to receive holistic care in their own home.Join today to help us meet the needs of patients and their families, supporting them to achieve improved health and wellbeing.
Main duties of the job Undertaking a Specialist Respiratory Nurse / Physiotherapist role in leading, planning and co-ordinating the delivery and evaluation of care across the Community Respiratory and respiratory admission avoidance services.Providing specialist Respiratory Nursing / Physiotherapy interventions to patients as part of the specialist teams, assessing and treating own caseload of patients who may have a complex presentation and maintain associated records as an autonomous practitioner.Meeting the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment.
Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.Deputising for the Respiratory Team Lead including co-ordinating the day-to-day work, influencing clinical decisions, and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise.Prioritising and undertaking initial assessments, providing specialist advice and expertise in line with the requirements set out in the HCPC / NMC code.Critical analysis of complex clinical data, such as arterial/capillary blood sampling to inform management and decision making.

Person Specification Qualifications Essential Professional UK registration HCPC / NMCDegree level education or equivalent knowledge and skills gained through any combination of alternative study or employment experiencePost registration or specialist training relevant to managing respiratory conditionsEvidence or portfolio of post graduate continued development Desirable Certificate/Diploma in Management and Leadership or relevant equivalent experience Experience Essential Evidence of Continued Professional Development (CPD) Experience of working with patients with respiratory disease and long-term conditions Experienced in assessment and developing patient care plans Experience in developing and delivering care and clinical interventions to patients Experience of staff supervision and mentorship Adhere to current statutory requirements, standards and regulations Awareness of local and national agenda within the speciality Desirable Experience of project management for policy development Additional Criteria Essential Skills Highly specialised clinical skills across the speciality Organisational and ongoing planning skills including own workload and of others as required Ability to prioritise and have good time management skills Excellent verbal and non-verbal communication skills and proactive listening skills Dexterity required for taking arterial/capillary blood sampling and oxygen assessments, as required for role.
Advanced communication skills including to communicate complex sensitive or confidential information in an appropriate manner, to liaise and negotiate effectively Understanding of effective clinical governance including implications, quality and audit Ability to recognise own clinical boundaries and when to seek advice Able to work at intense levels of concentration during most of the working day Knowledge Advanced knowledge of speciality Understanding of the health needs of people in our care and health promotion Understand the legal responsibilities of the profession Knowledge of, understanding and respect for the work of other health care team members and professionals Understanding of infection control procedures Understanding of the meaning of clinical supervision/mentoring
