Job summary This is an exciting opportunity for a Long Term Conditions Nurse to join Rosebank Health.
We are looking for an experienced, enthusiastic Respiratory Nurse to take an active role in our team, providing high quality care to our 40000 patients.The role is will be focussed on COPD and Asthma, reviewing a wide age range of patients and their respiratory health, managing acute and chronic needs.In addition there is potential for general treatment room duties.
Main duties of the job The successful candidate will:- Undertake the role of Respiratory Nurse and collaborate with colleagues in the planning and delivery of respiratory care in line with Practice need.- Work as an autonomous specialist practitionereffectively coordinating the delivery of an expert service within respiratory disease.
Supporting health and social careprofessionals in accordance with the local and national guidance, in order topromote guideline compliance.- Work closely with the wider nursing team, clinical admin team and Lead GP to meet quality standards such as QOF.
About us Rosebank Health Is a large well-established training practice of 40000 patients.
We operate from spacious, purpose-built premises on four sites in the Gloucester area.
We have a wide clinical team including GP Partners, Salaried GPs, Advanced Nurse Practitioners, Practice Nurses, Paramedics, HCAs, Phlebotomists, Pharmacists, Social Prescribers and a Physiotherapist, who are all well-supported by a large team of committed administrative staff.
Job description Job responsibilities Responsibilityfor Patient CareClinicallyassess, manage and review respiratory patients, identify and advise on riskfactors and act to prevent avoidable hospital admissions where possible.Responsibility for caseloads, focusing particularly on the complex needspatients with respiratory illness, ensuring all patients have an accuratemanagement plan, reflecting assessments and incorporating the issues andrecommendations made.Practiceas an autonomous professional in self-directed manner with confidence,authority and expertise, undertaking comprehensive range of clinical skillswithin specialist area.Promotehealth and wellbeing, empowering patients to make lifestyle changes and self-manage,offering the appropriate health education information to advice to patients,relatives and carers.Provideexpert nursing advice and support to patients, their families/carers followingdiagnosis and through treatment process, identifying particularly complexclinical needs.Useprofessional judgement and clinical experience in managing complex andunpredictable patient events.
Learning from situations to improve futurepatient care and service delivery.Criticallyevaluate and challenge current practice and processes of respiratory care andmanagement, identifying and initiating appropriate steps to assist withinnovative solutions to modernise and develop services to achieve Practiceobjectives.Help with the identification, development and evaluation of clinical protocols andstrategies to standards of specialist respiratory disease management whilstensuring equity of access into the service via numerous pathways/referralroutes.Collaboratewith IT to ensure full understanding of QOF recall systems and to ensure smoothrunning of clinics.Collaboratewith palliative care services and other professional disciplines to provide aseamless service for patients who are end of life.
Assist in the management of patients in theterminal phase by joint working with community/neighbourhood care healthcareprofessionals and palliative care services following end of life discussionsand offer appropriate specialist opinion in relation to the management of respiratoryillness.Assist to implement nurse led clinics for respiratory management using advancedspecialist skills and knowledge to ensure consistent high standards of care.Contributeto PCN meetings and Projects.Deliveringa Quality Service Recognise and work within own competence and professional code of conduct as regulated by the NMC Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Prioritise, organise and manage own workload in a manner that maintains and promotes quality Deliver care as an individual and Nurse Team according to NSF, NICE guidelines and evidence-based care Assist with the assessment of effectiveness of care delivery for the Nursing Team through peer review, benchmarking and formal evaluation Assist on the maintenance of quality governance systems for the Nursing Team Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, suggesting improvements where required Evaluate patients response to health care provision and the effectiveness of care Actively participate in shared learning across the Practice Understand and apply policies that support the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedures and local guidance Work within policies regarding family violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriateLeadership- Personal and People Development Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model Support the development of others in order to maximise staff potential Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Assist in the planning and implementation of changes within the area of care and responsibility Assist and participate in the development of local guidelines, protocols and standards where appropriate Promote the role of the Nursing Team in the provision of careTeamWorking Understand own role and scope in the Practice and identify how this may develop over time Delegate appropriately, adopting the principles of safe practice and assessment of competence of nurses Ensure clear referral mechanisms are in place to meet patient need Work effectively with others to clearly define values, direction and policies impacting upon care delivery Support nursing team activities that create opportunities to improve patient care Participate in and support local projects as agreed with the Nurse ManagerManagementof Risk Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines Ensure the safe storage, rotation and disposal of vaccines and drugs Act as a role model to support members of the Nurse Team to undertake mandatory and statutory training requirements Apply infection-control measures within the Practice according to local and national guidelines Apply policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all Participate in the local implementation strategies that are aligned to the values and culture of general practice UtilisingInformation Use technology and appropriate software packages as an aid to the planning, implementation and monitoring of care, presenting and communicating information Review and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes Manage information searches using the internet and local library databases Understand the responsibility of self and others regarding the Freedom of Information Act Collate, analyse and present clinical data and information to the Practice Team, (using appropriate visual aids etc.

Person Specification Experience Essential Experience working in a primary care setting.
Evidence of working autonomously or as part of a team.
Understanding and knowledge of healthcare provision in general practice, including QOF and enhanced services.
Desirable Good knowledge of clinical IT system such as Systm1.
Skills Essential Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face.
Knowledge of IT systems, including ability to use word processing skills, emails and the internet.
Ability to promote best practice regarding all nursing matters within your scope of practice.
Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
Effective time management and adaptability.
Demonstrate personal accountability, emotional resilience and work well under pressure.
Desirable Flexibility to work outside of core office hours.
Qualifications Essential NMC Registered Nurse.
Diploma Level or equivalent in Respiratory.
Desirable Educated to Degree Level.
Independent Prescriber Qualification.
