Main duties of the job Your role will include providing nursingcare, undertaking patient reviews and managing symptom control as well ascontributing to the development of our services to increase access for patients.You will joina bank of nurses who can work a minimum of 20 hours per month which willinclude weekend working, night duty and Bank Holidays.
The night shifts arefrom 10pm-7am and day shifts range from 1-4 hours.
Experience in community work, palliative and end of life care isdesirable but not essential and you must be able to fulfil the travel requirements of the job (public transport does not fulfil the requirements).This post would be ideal for anyone wishing to self-roster their availability to work.

Home based, travelling to patient homes.
Occasional travel to Hexham Office.Reports to: Deputy Head of CareHours: Bank, minimum 20 hours per month.General StatementTynedale Hospice at Home registered nurses will provide a minimum of 20 hours per month availability, to include weekends, Bank Holidays and night duty.
You will be part of the multi-disciplinary team providing supportive and palliative care to end of life patients.Although you have access to advice you are expected to work unsupervised.
However, you have advice and support when required: clinical and peer supervision are provided.

Your views and expertise will be welcomed in our inclusive organisation.The tasks, skills and duties of the Tynedale Hospice at Home Registered Nurse are described below;ClinicalAssessing,planning, implementing and evaluating care provision within your scope ofpractice.Maintainingconcise and accurate records of care delivery within care plans and Caring forthe Dying Patient documentation.Maintain aduty of care to patients, carers and colleagues through the exercise ofpersonal and professional accountability.Monitorand evaluate end of life care plans to measure progress and ensure theeffectiveness of any interventions.Communication/DocumentationWork with patients, their families and friends in a sensitive and flexible manner, demonstrating an awareness of the impact of death, dying and bereavement and recognising that patient priorities and ability to communicate may vary over time.
Manage unpredictable and/or volatile situations effectively.
Facilitate communication with clients for whom English is not the first language and those who have communication/language difficulties.
Understand the range of assessment tools and ways of gathering information and contribute to patient activity data.Assess pain and other symptoms using assessment tools, pain history, appropriate physical examination and relevant investigations.
Undertake/contribute to multi-disciplinary holistic assessment and information sharing.
Demonstrate an awareness and understanding of the legal status and implications of the Advance Care Planning process in accordance with the provisions of the Mental Capacity Act.
Show an understanding of Informed Consent and demonstrate the ability to give sufficient information in an appropriate manner.
Work sensitively with families and carers to support them as the patient decides upon their preferences and wishes during the Advance Care Planning process.Leadership, Supervision and AppraisalWork without direct supervision to deliver care to patients, seeking advice from specialist nurse or members of the Primary Health Care Team when appropriate.
Work in partnership with Community and district nursing teams, Macmillan nurses, GPs and other health professionals who refer into our services.
Take responsibility for personal learning and continuing professional development and contributing to the learning of others.Promote evidence-based practice through continued professional development and the sharing of best practice within the team and with others.
Appraise and supervise interventions performed by other members of the nursing team in accordance with Hospice policies.
Attend Supervision sessions and appraisal as required by the Hospice.
Promote health and safety, undertaking risk assessments as necessary Clinical Governance and Quality StandardsAssist in the development of clinical governance and quality frameworks through patient focused benchmarking, clinical audit and critical incident reviews demonstrating learning from multiple sources.
Implement and adhere to hospice policies.
