Job summary As a CommunityNurse working in Warrington you will be integral to the care of the patientsduring their time with Bridgewater Community Trust is focused onsupporting the Community and helping patients remain independent in their ownhome.
You will actively support the District Nursing team by undertakingresponsibility for a delegated caseload.
Undertake the assessment, planning andevaluation of patients care needs.
Liaise with the District Nurse team toensure information sharing in relation to patient care / changes to care planand the continuity of care and reviewing of complex care packages.
You will liaisewith all health care professionals and other agencies to ensure effectiveservice delivery.
