What the Job Involves As a Reablement Officer, you will deliver high-quality support to service users, empowering them to achieve their care and health outcomes.
Your role involves working collaboratively with the therapy team to implement reablement plans and providing assessments to prescribe necessary equipment for promoting independence.
You will design, plan, and support care plans, while developing effective relationships with service users and supporting their inclusion and participation in activities.
Although you have no direct supervisory responsibility, you will assist with the induction and on-the-job training of colleagues.
Working as part of the Reablement and Assessment and Prevention Service team, you will contribute to positive service delivery outcomes, uphold council policies, and ensure compliance with financial regulations and statutory requirements.
Your role requires flexibility to work a four-week rotating schedule, including one weekend in four, and the ability to respond to emergencies as needed, with mileage compensation provided for travel during working hours.
