Job summary This is an exciting opportunity for an enthusiastic and experienced Mental Health Practitioner to join our General Practice integrated mental health service (GPimhs) in Epsom PCN.This is a Full-Time ( Hrs - Monday to Friday 9am to 5pm) role.The post-holder will be working from the GP Practices within Epsom PCN and Horizon SABP Hub in Epsom.

There is additional support from specialist psychiatry and pharmacy.
Main duties of the job The successful candidate will take a key role in developing and shaping a major new and transformative approach to providing mental health care within the community.
They will be responsible for working closely with General Practitioners and the primary care team to provide assessments, brief interventions, and care navigation for people with significant mental health issues living in the locality.
The successful candidate will need to work closely with local mental health services to support an easy in and out approach between secondary and primary care.
As this role is based within a community setting we do require that you must be able to travel between multiple sites and trust locations within the set hours of your working day.

Person Specification Qualifications Essential Qualification in Mental Health Nursing, or Occupational Therapy or Social Work at Degree level or equivalent Professional Registration.
Professional registration such as NMC/HCPC Experience Essential Completion of mentorship / preceptorship course or the intention to complete to people with a variety of mental health problems and serious mental illness.
Experience of providing a range of evidence based interventions to people with a variety of mental health problems and serious mental illness and experience working in the community, psychiatric liaison or of confidently working independently with people who experience severe and enduring mental illness
