The practice nurse will be responsible for a number of clinical areas such, health promotion, chronic disease management, health prevention, well women/well man clinics, as well as routine practice nurse duties.
Supporting the management team in the reviewing of clinical policy and procedure.
Vaccinations - Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions (essential) Long Term Conditions Management Asthma, Diabetes, COPD, CVD, Learning Disabilities, Anticoagulation monitoring (essential) Travel Health - Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, food hygiene, sun protection.
Health Promotion & Screening Cervical Smears, Sexual Health & Family Planning, Contraception pill checks/coil checks.
Well Person Clinics To provide a holistic assessment and lifestyle advice on diet, smoking, alcohol intake & exercise.
Perform venepuncture according to local guidelines.
Assist in the provision of minor surgery and coil clinics Ensure infection control guidelines are maintained.

Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
