The Role We are seeking a passionate Practice Nurse to join the team, as a vital part of our patient-focused team, you will play a key role in delivering top-notch primary care, working alongside a skilled multidisciplinary team to meet the health needs of our patients and contribute to service improvements.
What You'll Do Provide high-quality patient care within a collaborative and supportive team environment.
Manage chronic disease cases and offer specialised support in areas like diabetes care and long-term conditions.
Conduct first-contact assessments and provide telephone triage.
What We're Looking For Qualified Practice Nurse with a minimum of two years' experience in a primary care setting.
Proven background in chronic disease management.
Exceptional teamwork, communication, and interpersonal skills.
Experience with long-term conditions is desirable.
