Job summaryNorthlands Surgery are looking for amotivated and enthusiastic Practice Nurse to join our friendly, patient focusedpractice team.
The Practice Nurse will beresponsible for a number of clinical areas such as infection prevention andcontrol, health promotion, chronic disease management, immunisations, cervicalscreening, as well as actively supporting the practice management team in thereviewing and delivery of clinical policy and procedures.The Practice nurse will also beresponsible for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and housebound patients.Main duties of the jobThe main duties of the role are:Chronic disease management including the diagnosis,monitoring and development of patients individual management plans.Health promotion and wellbeing.Implement and evaluate individual treatment plansfor chronic disease patients.Identify, manage and support patients at risk ofdeveloping long-term conditions, preventing adverse effects on patients' health.Provide routine nursing care to patients asrequired in accordance with clinical-based evidence, NICE and the NSF.Provide wound care (ulcer/Doppler etc.)
to patients.Provide immunisations (childhood, seasonal etc) topatients.Provide travel medicine services.Provide Cervical Cytology screening.Request and process pathology results as required.Maintain accurate clinical records in conjunctionwith existing legislation.Ensure read codes are used effectively and supportwith achievement of QOF targets.Chaperone patients where necessary.Support the team in dealing with clinicalemergencies and safeguarding matters.Support patients in the use of their prescribedmedicines or over-the-counter medicines (within own scope of practice).Liaise with external services/agencies to ensurethe patient is supported appropriately (vulnerable patients etc.).
Person Specification Qualifications Essential NMC Registered Nurse Evidence of post registration training.
Continued personal development.
Desirable Experience/Diploma in a Chronic Disease Management area Diabetes, CHD, COPD, Asthma.
