The Honiton, Ottery St Mary, Sidmouth, Axminster and Seaton Cluster (HOSM-SAS) has an attractive mix of rural villages and coastal towns whilst being only -mins drive from Exeter.
The Cluster is set in a landscape of outstanding natural beauty.
The market towns of Honiton and Axminster provide excellent rail links to London and Exeter.
The post will be based in either Seaton, Axminster and Sidmouth depending on the successful applicant.
The emphasis will be on providing clinical leadership to ensure the delivery of integrated rehabilitation pathways across community services; for patients living in their own homes, to avoid hospital admission, supporting timely discharge from hospital and interventions for those placed in Pathway 2 beds to return home.
The Community Rehabilitation team support the Ageing Well rota to ensuring the provision of a 7-day 8-8 service.
Rota co-ordination and allocation is provided by the Band 7 in conjunction with the UCR Lead and other Band 7 colleagues.
Knowledge and experience of rostering would be helpful.
The role will support the B7 OT clinical Lead and Therapy Manager with triaging and clinical decision making and provide specialist clinical supervision to the team to ensure effective day to day operational and clinical management of the therapy Service for the Cluster.
You will support staff to manage complex caseload and support colleagues through mentoring, coaching and teaching sessions.
The successful candidate will liaise and work closely with Community & Specialist Nursing Colleagues, General Practitioners, community hospital and the acute hospital, as well as with other therapists within the Integrated Health Team.
