Job summary An exciting opportunity has arisen to work as aPharmacy Technician in Lincolnshire to support GP practices within the PCN.
This position will be based at Caistor Health Centre.
Weare looking for an enthusiastic, highly motivated Pharmacy Technician with aminimum of 2 years experience to join our existing PCN pharmacists team.
Thesuccessful pharmacy technician candidate will play an important role incomplementing our clinical pharmacists and other members of the PCNmulti-disciplinary team.
Main duties of the job The post holder will work withintheir clinical competencies as part of a multi-disciplinary team to provideexpertise in clinical medicines management, provide face to face structuredmedication reviews, manage long term conditions, management of medicines ontransfer of care and systems for safer prescribing, manage repeat prescriptionauthorisations and reauthorisation, acute prescription request, whileaddressing both the public health and social care needs of patients in the GPpractice(s) that make up the PCN.The post holder will perform face to facemedication review of patients, especially for older people, people inresidential care homes and those with multiple co-morbidities.
The post holderwill provide support on quality improvement and clinical audit and well asmanaging some aspects of the Quality and Outcomes Framework.
