Job summary Chorley Central Primary Care Network are looking for an experienced HCA to join the PCN Care Home team.You will be part of an established team delivering the Enhanced Care in Care Home (EHCH) service for the 16 Care Homes in the PCN Main duties of the job Working under the direct supervision of the PCN Nurse andstrictly in accordance with PCN EHCH Guidelines and protocols.Your duties will includeConducting 6 monthly care plan reviews.Ensuring all residents have a Personalised Careand Support Plan (PCSP)Administer Influenza and COVID vaccinationsunder appropriate PSD (vaccination training will be provided)Working within and supporting amultidisciplinary team (MDT) approachPhlebotomy is desirable but not essential (trainingcan be provided)Hours are 9am -2pm Tuesday - Friday About us Chorley Central Primary Care Network (CCPCN).

Excellent interpersonal, verbal and written communication skill Car driver/access to car Desirable Experience of using a medical software package Qualified in Phlebotomy Knowledge Essential Understanding of systems to gain an understanding of the health needs of the population as they relate to primary care Understanding of evidence based practice Knowledge of national standards that inform practice ( National Service Frameworks, NICE guidelines etc) Desirable Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework
