Job summary The Acute Advanced PractitionerTeam of 2 APs will work to provide additional acute access for the PCN memberpractices and will work as part of the wider PCN multi-disciplinary team.
Therole requires the practitioner to be able to assess and treat patients withintheir scope of practice as well as close liaison with the patients GP practice,and to be an independent prescriber.This is an excitingopportunity to be part of and help develop a new service from its inception.Working hours will be within practice contract hours of 8am and , andspecific working patterns can be discussed with the successful candidate.
Main duties of the job Recognise and work within own competence andprofessional code of conduct as regulated by the NMCProduce accurate, contemporaneous and complete records of patientconsultation, consistent with legislation, policies and proceduresPrioritise, organise and manage own workload in a manner that maintains andpromotes qualityDeliver care according to NSF, NICE guidelines and evidence-based careAssess effectiveness of care delivery through self and peer review,benchmarking and formal evaluationInitiate and participate in the maintenance of quality governance systems andprocesses across the organisation and its activitiesUtilise the audit cycle as a means of evaluating the quality of the work ofself and the team, implementing improvements where required Inpartnership with other clinical teamsCollaborate on improving the quality ofhealth care responding to local and national policies and initiatives asappropriateEvaluate patients response to health care provision and the effectiveness ofcareUse a structured framework (eg root-cause analysis) to manage, review andidentify learning from patient complaints, clinical incidents and near-misseventsAssess the impact of policy implementation on care deliveryUnderstand and apply legal issues that support the identification ofvulnerable and abused children and adults, and be aware of statutorychild/vulnerable patients health procedures and local guidance About us Leigh Primary Care Networksupports is made up of 9 practices in Leigh (Wigan Borough) covering a patient population ofroughly 57,000.Leigh PCN memberpractices have worked together successfully for a number of years, but theformation of PCNs has allowed the PCN to develop further and establish asuccessful workforce including First Contact Practitioners, Care Coordinators, Community Link Workers, and Pharmacists, led by an effectiveleadership team headed by our Clinical Director and Data & TransformationLead.Allthese services are successfully embedded within our practices.
We now want to add an ANP service to the teamand believe this will be instrumental in continuing to meet the challenges inprimary care and providing high quality and responsive services in Leigh.
